CCYH Scholarship Application For High School or Prep School Seniors
CCYH Scholarships
Scholarships of $1,500.00 each will be awarded to at least one deserving CCYH alumni boy and girl who have made positive impacts on the Concord Carlisle and Hockey communities.
Scholarship recipients will be notified around the end of June each year.
Eligibility
1. Senior in a Massachusetts High School, or a Massachusetts resident attending a Prep school
2. Member of CCYH core program team(s) (U8, U10, U12, U14, or U16/U18) for at least three seasons (past or present)
3. Plan to attend a four-year, two-year, or vocational school full-time.
Instructions
1. All Applications must be postmarked prior to June 1 of the scholarship year and should be mailed to:

2. Application must be typed or printed neatly.
3. Provide three (3) recommendations from people that are not members of your family. Use the forms provided.
4. Submit, sealed high school transcript (3.5 years) with application.

1. Academic status
2. Number of years as a member of CCYH
3. Scholastic Honors/Awards
4. Curricular and extracurricular activities
5. Positions of leadership held
6. Community related activities
7. Letters of recommendation
8. Applicant essay
(Points will be awarded for number of years in each activity)
